1) Please fill out the form electronically using the text boxes provided for:
- Name
- Instrument/Major
- Major Professor
- Repertoire to be performed
2) Select class rank (and jury type as appropriate) by using the click buttons below the selections. Use the "Undo" function to correct click button errors.
3) Students are to fill out the form electronically, print one copy of the completed form for each faculty member in their applied area, and provide it to the faculty members at their jury.
