The forms below are available to report incidents involving property loss, damage, or non-employee injuries.

MSU Property Loss Report

Use this form to report MSU-owned property that has been damaged, destroyed, or stolen. Submissions should be completed within 90 days of the incident to be considered for potential insurance coverage.

Injury/Property Damage Report

Use this form to report incidents involving non-employees that result in bodily injury or property damage, excluding motor vehicle accidents.
