College of Music Workplace Injury Reporting Procedures
1. Report the injury:
- Use the Report of Claimed Occupational Injury or Illness form.
- Complete as much of the form as you can and be sure the employee signs and dates it.
- Submit the form to the on-site HR Director, College Administrator, Faculty Member, or Supervisor.
- If you’re unsure who to contact or no one is available, email it to College of Music HR Inbox at music.hr@msu.edu.
- The HR Director will review the form, obtain required College signatures, and forward it to Central HR.
2. If medical treatment is needed (now or later):
- Download the Authorization to Invoice MSU form and have the injured employee take it to a listed provider (Lansing Urgent Care or Sparrow Hospital ER).
- This form ensures the employee is not billed for the visit.
- If treatment is outside the Lansing/East Lansing area, use Authorization to Invoice MSU – Employee Outside of Lansing Area.
