Jury Report Form and Instructions

To ensure a smooth and efficient jury process, please follow these steps when completing the Jury Report Form (fillable PDF).

1. Complete the Form Electronically
   • Use the provided text boxes to fill in:
     • Your Name
     • Your Instrument/Major
     • Your Major Professor
     • The Repertoire you will perform
2. Select Class Rank and Jury Type
   • Use the click buttons to indicate your class rank and the appropriate jury type.
   • If you make a mistake, use the “Undo” function to correct your selection.
3. Print and Distribute Copies
   • Once the form is completed electronically, print one copy for each faculty member in your applied area.
   • Bring the printed forms with you and provide them to the faculty at your jury performance.
