Permissions & Enrollment Forms
Late Enrollment (Active for Spring 2026):
Please complete the College of Music Late Registration Request Form if you have permission to join a course after the 5th day of the semester. This form WILL enroll you in the course if completed correctly. ALL submissions MUST include a record of INSTRUCTOR PERMISSION. When applicable, the record should also acknowledge:
- Prerequisite permissions
- Audition results
- Courses outside your major/minor requirements, and are not typically offered as electives.
- Private lessons on a secondary instrument/voice.
Permissions (Active for Summer & Fall 2026):
Please complete the College of Music Permissions Form for permission to register for a restricted course. This form will NOT enroll you in the course. Students will still need to register manually.
Many submissions require INSTRUCTOR APPROVAL. Common reasons to include a record from a faculty member include, but are not limited to:
- Prerequisite permission.
- Courses outside of your major/minor requirements and are not typically offered as electives.
- Private lessons on a secondary instrument/voice.
